Office policies

When you visit us

We are pleased that you have chosen us to provide your medical care.

Please bring the following to your appointment:

• A current copy of your insurance card/cards.
• A list of your current medications and refill status.
• The name of the pharmacy where you wish to have your prescriptions filled.
• If this is your first visit, bring completed registration forms.

Appointment and cancellation information

We encourage you to call in advance as far as possible to schedule "routine" appointments. We offer same-day urgent or "sick" visits. The office opens at 8:00 am. Some evening hours are available for your convenience.

If you are unable to keep your appointment, please call the office as soon as possible. This courtesy allows us to better serve all our patients.

• Please give a 24-hour notice for cancellations.
• If you are late for your appointment, you may be asked to reschedule for another day.
• If you are not present for a scheduled appointment, you may be charged a fee.
• Repeat no-show patients may be asked to find another primary care physician.

Payment information

Payment is expected at the time of service. This includes your co-pay or any co-insurance that you may have. We accept cash, check, debit and credit cards. If you have questions regarding payment options or insurances, please call us at 207-363-8430 ext. 24.

Referrals

If your policy is referral-based, it is your responsibility to know which services require a referral or pre-certification and if your specialist is in your plan. Always check with your insurance company first if you have questions concerning the referral process, especially if you have made appointments outside of this office on your own. We will make every effort to process your referrals promptly but cannot guarantee a last minute referral.

Prescription renewal

• Please give 24 hours of notice for prescription refills.
• Please do not wait until you are out of medication to call.
• Emergency requests for prescription refills will be limited to seven days.
• Controlled medications will not be renewed after hours.

Medical records

There is a fee associated with copying medical records. The charge is $10 for the first page and $.25 per page thereafter. The records will not be released prior to payment and proper release authorization. Please allow 2 weeks turnaround.
